This position will be responsible for assessment, planning, communication, education, resource management, and service facilitation for the HCSB Case Management Program. This position links clients with appropriate providers and resources.
This position is responsible for conducting telephone screening interviews to determine potential client eligibility for home and community-based services. The position also provides information and makes referrals to all other appropriate aging services.
